Physical Examination & Oral Health Assessment Requirements

Physical examination for any TK, kindergartener or new first grader is NO longer required for school entry as of July 1, 2024.

The Kindergarten Oral Health Assessment Form OR Waiver Form is still REQUIRED for the 2024-25 school year.

Complete this form at your School's paperwork day or it can be found here: Confidential Health History Form

This form includes important health information that helps the school nurse and school site keep your student safe and healthy.

Include any allergy information, health conditions that are managed during school hours (asthma, epilepsy, etc.), and medications that need to be taken during school hours.

Medications at School: Any medication, including inhalers and over-the-counter medications require specific documentation completed by a physician.

Certain health conditions will need a health action plan completed by a physician.
